Fees

Co-payments, deductibles, and co-insurance amounts are all due before service is rendered. We will verify your benefits and calculate a good faith estimate prior to your arrival. It is your responsibility to know your benefits. We cannot re-verify benefits upon your arrival to the office.

Payment is expected at check-in unless prior financial arrangements have been made. We accept all major credit cards.

When your insurance has processed your claim, any remaining amount you owe will be billed to you. If you need to make special arrangements concerning your bill, please call the office prior to your appointment. Please remember we are filing your claim to your insurance as a courtesy. Insurance companies are varied and complicated. We expect your assistance and payment if your insurance denies your claim.

We understand that health insurance benefits are often very frustrating and confusing and this can lead to a misunderstanding or conflict between our office and our patient. Please keep us updated on any insurance change and please be kind to our staff. We are the intermediary between you and your insurance company.

Common Out of Pocket Fees, not billed to insurance, for our office include:

  • Growth Hormone Administrative Costs Fee - 100.00
  • School forms/Letters/Care plans Fee - 25.00
  • FMLA - 100.00
  • Stimulation Testing Fees - Fees are calculated once the provider has decided this testing is needed and will include a 250.00 out of pocket testing/facility fee that is not billed to your insurance in addition to insurance codes that are applicable to the visit.

Missed Appointments:

Because a missed appointment squanders an appointment time when another patient could have been seen, as well as delaying your child’s care, our office requests a 48-hour notice of a cancellation. As a courtesy, an appointment reminder call will be made in the days prior to your child’s scheduled visit.

Failure to cancel an office visit appointment 48 hours before the date and Stimulation test 7 days before the date will result in the following fees :

  • Missed New appointment Fee - 50.00
  • Missed Follow Up appointment Fee - 25.00
  • Missed Stimulation testing appointment fee - 150.00 (4 hour time slot)

Insurance

We are currently accepting most commercial insurance plans. If you have any questions concerning coverage, please contact our office or your insurance company.

Please remember to bring your insurance card to each visit so we may file your insurance properly. For insurance providers with which we are not contracted, you will be given a copy of services and charges to file with your insurance claim form. Please keep in mind that providers are too numerous and varied for our staff to know all details of all available plans. It is your responsibility to know your terms and coverage for your own insurance contract.

We do not accept any of the CMOs including Amerigroup, Peachstate, Wellcare, Medicaid, CareSource and do not accept any MarketPlace plans at this time.

If you are late for your appointment:

Please call us if you do not think you will be on time for your child’s visit. Your time is valued by us, and we make every effort to see your child on time. Often our providers are running late because a prior patient arrived late. We have a 10 minute grace period for your appointment, but at your doctor’s discretion, you may be asked to reschedule your appointment.

Medical and Camp Forms:

Sports physicals should be completed with your child’s primary care physician. We are happy to review and sign forms related to your child’s endocrine condition. Please complete as much of the form as possible prior to your visit. If a form is dropped off, mailed, or faxed to us in-between office visits there is a fee associated with the completion of the form and we will make every effort to return the form within 7 days.

Prescription Refill Requests:

To minimize the need for a refill request between office visits, we will make every effort to provide you with an updated prescription at your visit, and the prescription will typically have an adequate number of refills until your next appointment. Please send us a portal message or call the office directly to request a refill. Please call your pharmacy prior to calling our office to ensure that you do not already have available refills left at the pharmacy.

We do not accept electronic or faxed requests directly from the pharmacy as many are "auto-populated" and are not requested by the patient. Please note that you must have a scheduled upcoming appointment with us in order for our staff to renew your request. We will make every effort to refill prescriptions within 2 business days.

We do not provide growth hormone refills in between appointments as refills are provided at the last appointment to last until your next visit.

No prescriptions will be filled after hours or on weekends unless absolutely necessary. If you have not been seen at our office because you have failed to make an appointment or if you have failed to show for your last appointment, we reserve the right to refuse to prescribe medication or to issue a time-limited prescription, to allow you to make another appointment. This is in accordance with the principles of good clinical practice.
